how to fill the request form

2 -Weight - Height: according to weight and height it is calculated the Body Mass Index BMI from which the length of the training program is established, so that the increase of the load between the individual sessions is built on the real morphology features of the user.
3 -Medical certificate date: before starting any restart program it is recommended to submit you to thorough physical examination and in particular to consult the
Screening questionnaire to pre participation in physical exercise programs,
4 -Password - code to enter as your password - In absence of password, you can continue and activate the on-line payment at the end of the form, completing with the BILLING DETAILS.
5 -The Cooper test: enter the distance [m] you can cover in 2 minutes (also walking very slowly) for programs deambulation or 6 minutes for programs running.
- The minimum value accepted for programs deambulation is 100 m. For values below 100 m is recommended that a further course of physiotherapy hospital
- The minimum value accepted for programs running is 400 m.
The level of the program to be proposed is determined from the test result.
Warnings

2. Before starting the training program it is recommended to submit you to thorough medical examination;
3. Ask, and perform a cardiac stress evaluation;
4. Do not give up your program to another person;
5. Perform training with heart rate monitor;
6. Slow down if you come to a state of fatigue higer than the heart rate indicated below, which
according to your BMI corresponds to the % of your theoretical maximum heart rate (220 - age for men and 210 - age for women);
7. Do not persist until the end of training session, if you consider the state of fatigue (which may ave occurred) excessive and persistent. If this should repeat for several times in a row (2 or 3) , report it to the sport-sys.it that will change the program.
8. If training is not completed , it is advisable to repeat it before moving on to next training;
